Our mission is to offer you or your loved ones all your health care needs in a home-like setting. We have combined all of our services to Assure that all of your daily needs are met. Assuring Possibilities, Inc. is here to help individuals in need of a place to stay. Fortunately, there is a group of people who are here for you. We provide housing for male and female adults,  Veterans, homeless and those with mental illness. With our home-like environment you will be happy you chose us. Assuring Hearts Med Pass does not only focus on medication administration; we also know the importance of binding and building trust with our residents and their families. We provide patient and family education on medications adverse reactions and the different signs and symptoms to be aware of while caring for your loved ones. In addition, we have on-call nursing staff for any questions you may have. Assuring Hearts has individualized contracts suitable for you and/or your loved ones needs.

Assuring Hearts Med Pass is here to provide patients with safe medication administration at affordable prices. With Assuring Hearts Med Pass' trained, professional, and

hand-picked licensed professional nurses, we can assure you that your residents will receive quality individualized care.

Assuring Hearts Med Pass Services include but are not limited to:

​

  • Customized contracts designed to fit each patient's needs

  • Administering medications twice a day AM and PM, 365 days a year   (noon per request)

  •  Documentation of all medications given on individualized Medication Administration Records (MARs)

  • Assessment and documentation of vital signs as needed/ Blood Glucose monitoring

  • Refilling of prescriptions

  • 24 hours a day Patient/ Family education

  • Medication reminders/ pill case refill (in home or facility)
